DHA Reached the Brain, But Missed the Memory Memo
The omega-3 got into spinal fluid just fine. Your brain still didn’t budge.

What the Trial Tested
DHA marched straight into the brains of older adults at risk for dementia, but it still didn’t flex on memory or brain scans. A 24-month trial found that high-dose omega-3s boosted a key cerebrospinal fluid marker, yet the big payoff never showed up. Science really said, “Nice try.”
Researchers gave 365 adults ages 55 to 80 either 2 grams a day of DHA or a placebo for two years. Everyone started with low dietary DHA and at least one dementia risk factor, and nearly half carried APOE ε4, the major genetic risk tied to Alzheimer’s. The team split people into a lumbar-puncture group and a no-puncture group so they could check brain fluid directly without forcing everyone into spinal-tap club.
The main question was simple: could DHA raise the CSF DHA-to-arachidonic acid ratio, a sign that the nutrient actually reached the brain? Yes, and the effect was big. At 6 months, the DHA group’s CSF DHA/AA ratio rose by 0.17, while the placebo group slipped by 0.02.
What the Study Found
That difference held steady whether someone carried APOE ε4 or not. In plain language, the supplement made it to the brain side of the body just fine. The red blood cells also showed the omega-3 boost, so the pills were definitely doing something.
But here’s the bummer: over 24 months, DHA did not beat placebo for hippocampal volume, cortical thickness, or cognitive scores. Memory on the RBANS test improved a little in both groups, which is what often happens when people keep getting tested and stick around long enough to become familiar with the drill. The brain marker moved, but the real-life results did not.
Why This Matters
This study matters because a lot of supplement research gets stuck in the same mess: you can swallow the thing, but did it actually reach the target? Here, the answer was yes for the brain marker, which makes the null clinical result harder to wave away. It suggests that for this group, DHA shortage was not the main reason memory and brain structure did not change.
The authors say the field may need to stop asking only whether more DHA helps, and start asking how the brain handles DHA in the first place. That could mean looking at metabolism, mixing DHA with other interventions, or testing people who already show earlier signs of neurodegeneration. In other words, the puzzle may be bigger than just popping more fish oil like it’s brain DLC.
What to Keep in Mind
This was a phase 2a trial, not the final boss of dementia prevention. It also had a 38% dropout rate, mostly because COVID-19 wrecked the follow-up schedule. Still, the safety picture was pretty chill: side effects were similar in both groups, and no serious events were blamed on DHA.
Bottom Line
High-dose DHA did reach the brain in this trial, but it didn’t translate into better memory or bigger brain structures over two years. So yes, the supplement moved the needle in the lab — just not in the way people hoped. If you’re thinking about omega-3s, this study says “biologically active” is not the same as “brain-saving.”
